The following are the provider codes that identify the provider type to be used for payment and the descriptions of provider types on the payment screen:

	AR	=	Approved relative providers
	CT	=	Center providers
	IN	=	In-home providers
	NF	=	Non-relative in family day care providers
	NG	=	Group care providers
	RF	=	Relative in family day care providers
	SC	=	Self-declaration providers
	TR	=	Tribal Registration providers

 

When licensing or registering a provider or when a provider is approved as self-declared, Children and Family Services assigned a type to a provider. The following is a listing of the provider types that can be viewed when inquiring on a provider:

				C	-		Child Care Center		X	K	-	School Age Child Care
				E	-		Preschool Education Facility		X	M	-	Multiple License
				F	-		Family Child Care		X	P	-	Public Approvals
				G	-		Group Child Care Home		X	R	-	Tribal Registration
				H	-		Group Child Care Home			S	-	Self-Declaration
				I	-		In Home Child Care		X	Q	-	Approved Relative

 

The following is a listing of the out-of-state and Tribal numbers. These numbers are used in place of the county numbers in the license number.

		91	-		Minnesota	x	96	-	Turtle Mountain
		92	-		South Dakota	x	97	-	Spirit Lake
		93	-		Montana	x	98	-	Standing Rock Sioux
		94	-		Trenton Indian Service Area	x	99	-	ND Air Force Bases
		95	-		Three Affiliated Tribes
